Preliminary Proposal Form

Preliminary proposals may be submitted by downloading the Word document template and sending it via e-mail as an attachment to: emorales@hfsf.org. Please write: "Preliminary Proposal" in the subject line of the e-mail. Preliminary proposals may also be submitted by mailing two hard copies of the completed template form to the Foundation office. Please use standard 8-1/2 x 11 paper and a font size of 12.
